COMPLETE FORMS
Upon completion of registration, you will receive an email with your login and/or payment details. Use the details to login and fill in the following details:
- Medical history of the person you are enrolling.
- Current list of medicines being taken (if available).
- Available Dates for Master Health Check Up.

GAIN ACCESS
Once you are a registered user, you gain access to:
- First Master health check up results and reports of your parents.
- Routine Check up results and reports.
- In case of specific treatment / hospitalization, respective reports / discharge summary shall be updated.
